Creating a Survey or Quiz
 
Step 1
Click Site Actions
Click Create
Click Survey
 
Step 2
Enter a name and description for the survey or quiz
Select "Add survey to the quick launch"
Select show username in the results if this is not an anonymous survey Select no to "Allow multiple responses"
 
Adding Questions
 
Enter your question
Select the type of question (multiple choice, text, yes/no)
For each type of question, different options about how to represent the questions will be presented.
Click "Next Question" to add another question
Click "Finish" to exit adding questions when you are done
 
Setting Permissions for Surveys
 
Click advanced settings
Under Read Access
If you want any student to see all the results select "All Responses" otherwise select "only their own".
 
Edit Access You can specify which items a student can edit. It is recommended you their select "only their own" if you don't mind them changes responses later or "none" if you want to keep the results as is once submitted.
 
It is recommended that survey results not be allow to be added to the site search. 
 
Setting Permissions for Quizzes
 
Click advanced settings
Under Read Access Select "only their own".

Don't enable search for the survey.
 
Export Results to Excel
 
Click "View all site settings"
Select the survey or quiz
Under actions, Select "Export to Spreadsheet"
